Endoscopic Redo-Mitral Valve Replacement for hemi paralyses patient

New Delhi, February 07, 2022: Mr.Mahinder Singh,56years, living in Mount Abu, was referred to Shalby Hospital Jaipur for Minimally Invasive Mitral Valve Replacement by his Cardiologist.

He was complaining of dyspnea, PND and Palpitation from last 6 months which aggravated from last one month.

He was a known case of RHD,Severe MS,Moderate MR with Atrial Fibrillation and LVEF 40 %. He had undergone Closed Mitral Valvotomy (CMV )18 years back and Balloon Mitral Valvotomy 10 years back (BMV). 3 years back he had CVA and Left sided Paralysis. From which he was recovering and able to walk with support and does his daily activities himself with a little assistance.

His CT chest was showing gross pericardial adhesions. 2D Echo was suggestive of Severe MS,Moderate MR,severely calcified and thickened Mitral leaflets with fused, shortened chordae’s,Sever PAH, LVEF 40%.

Redo-heart Surgery is always clinically challenging due to elevated risk of Peri operative morbidity and mortality. Dense pericardial adhesion and complex valve exposure make the Re-do valve surgery challenging through medial sternotomy and Bleeding and prolonged surgical time increase the post operative risks.

After all necessary preparations, we have performed Totally Endoscopic assisted Minimally Invasive Mitral Valve Replacement for this patient. The heart was accessed through a 2.5-inch right thoracotomy incision and Endoscope was used for better visualization of the surgical field. A 27mm ST. Jude mechanical Mitral valve placed at the mitral position. Posterior Mitral leaflet and total chords were preserved.

The intra-operative and post operative period was uneventful, mobilized the patient on 1st POD and discharged from the hospital on 4th post operative day.

The patient and attenders were highly satisfied because he could do all his daily physical activities that he used to do before the surgery more comfortably and confidently at the time of discharge.

In minimally invasive heart surgery, we avoid cracking the chest thus significantly shorten the recovery period. We make an incision of 2 inches or less and reach the heart through the ribs without cutting any bones. And using an endoscope enables better visualization of surgical field, thus the incision can be kept minimum and over stretching of the ribs can be avoided.

  • The hospital stay lasts only 3-4 days unlike the conventional heart surgery that takes 6- 7 days.

Minimal invasive Cardiac surgery is developing into the new gold standard. But, Unfortunately, far too many people don’t know about minimally invasive heart surgery. This is partially because these procedures are new, so only a few doctors perform them. In fact, fewer than 1 percent of aortic valve surgeries worldwide are performed this way, as well as fewer than 20 percent of mitral valve surgeries.
